Immunizations

Services 

County Health Departments are responsible for providing recommended or required immunizations to all children birth through 18 years of age and to certain adult clients free of charge. These immunizations are provided along with other routine services or through immunization clinics conducted by the County Health Department. County Health Department staff also follows up on newborns at risk for being under immunized to ensure that they begin their immunizations. This is achieved through phone calls and mail. County Health Department staff is also responsible for vaccine preventable disease outbreak reporting and control.

The Florida Vaccines for Children (VFC) program is a federally administered and funded by the Centers for Disease Control and Prevention (CDC) through the National Center for Immunization and Respiratory Diseases to provide vaccines to children at no cost to the parent/guardian. In addition to local health departments, area physicians who immunize children or adolescents may also participate in the VFC program.

The Health Department can issue the form DH 680, Florida Certificate of Immunization or "Blue Card." This form is required for any child entering Florida schools (public or private, K-12, pre-schools, childcare facilities or family daycare homes). You can take your child's immunization records to the Health Department and if current we can complete the Blue Card for you. If your child's immunizations are not current, we can provide the needed immunizations or try to locate the most up to date information from another provider's office.

Immunizations are available Monday through Friday from 8:00am to 5:00pm. Bring all current immunization records with you. For more information call 352-498-1360.
